Qualifications

• Minimum two years’ Nurse Practitioner experience.
• Specialty in Family or Adult Medicine preferred.
• Experience with Part B billing.

Responsibilities

• Deliver comprehensive, patient-centered palliative care.
• Perform in-depth patient assessments.
• Develop and implement individualized care plans.
• Manage and adjust treatment plans based on patient progress.
• Prescribe medications and order diagnostic tests as allowed.
• Maintain timely clinical documentation.
• Participate in call rotation as needed.
