Job summary

Cornerstones of Care is seeking a Lead High Acuity Case Management Specialist to strengthen foster care services for youth and families. You will mentor and coach a small team while reviewing cases, conducting audits, and guiding program improvements.

The role involves analyzing family needs using Signs of Safety (SOS) and managing a 10–12 youth caseload under the Foster Care Contract.

Qualifications

  • Bachelor’s degree in social work, criminal justice or other human services related field.
  • MSW with Kansas licensure preferred; Behavioral Sciences Regulatory Board is recognized.
  • At least 21 years old with background check, physical and drug screening.
  • Valid driver’s license, proof of insurance, and reliable transportation.

Responsibilities

  • Provides mentoring, on the job coaching, and training to up to 4 staff (2 specialists, 2 interventionists).
  • Assists with peer record reviews, documentation audits, and evaluates reports for contractual compliance.
  • Serves on program and system improvement committees and participates in planning and development.
  • Analyze each family and child using SOS to develop resource and service recommendations.
  • Carries a caseload of 10–12 youth under the Foster Care Contract.

We are seeking a Lead High Acuity Case Management Specialist to join our team.Starting Salary: $57,000 - $59,000 (Salary)We are seeking a Lead High Acuity Case Management Specialist to join our team. Your role will be responsible for improving service delivery of the Foster Care Case Management services provided to youth and families. You will support the Manager of Permanency Services in the development of high acuity case managers and behavioral interventionists through training and coaching and will provide additional quality assurance through periodic case reviews, case consultation, and review of AVATAR documentation. As a member of the KS Foster Care team, you will work with other team members and report to our Manager of High Acuity Permanency Services.WHAT YOU WILL DO:Provides ongoing mentoring, on the job coaching, and training to a maximum of 2 high acuity foster care case management specialists and 2 behavioral interventionists.Assists with peer record reviews, completes documentation audits, and evaluates reports to ensure contractual requirements are met.Serves on program and system improvement committees and takes an active role in strategic planning and organizational development.Continually analyze and assess each family and child situation on an individual basis, using advanced knowledge in the area of child welfare to develop recommendations regarding supportive services and resources that each child and family may need, such as educational plans, medical, psychiatric and psychological assessments, therapy, independent living skills, etc.Assumes responsibility for managers, in their absence, as needed, directed and required.Carries a caseload of 10 – 12 youth who have been referred under the Foster Care Case Management Contract utilizing Signs of Safety (SOS), to include youth who are deemed ready to discharge from the High Acuity program.WHAT YOU WILL BRING:Our ideal candidate will have at least 2 years of experience in child welfare arena and the following:Bachelor’s degree in social work, criminal justice or other human services related field.Master’s degree in social work and licensed by the Behavioral Sciences Regulatory Board to practice in Kansas are preferred.At least 21 years of age and pass background check, physical, and drug screeningA valid driver’s license in the state you reside in, proof of current vehicle insurance, and reliable transportation.WHO WE ARE:Cornerstones of Care is a mental and behavioral health nonprofit certified in trauma-informed care that provides evidence-based prevention, intervention, treatment, and support services to help children and families improve their safety and health by making positive changes in their lives.
