Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home and senior living community care in Lewes, DE. You will conduct evaluations, craft plans of care, and deliver evidence-based therapy tailored to older adults in their homes or communities.

You’ll work with a multidisciplinary team, document precisely in FOX’s EMR, and may incorporate telehealth to extend access and continuity of care. This role emphasizes autonomy and meaningful patient outcomes.

Qualifications

• Valid Speech Language Pathology license in the state of practice.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in-home or in senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
