Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in private residences and senior living communities in North Chesterfield, VA.

You will evaluate, develop individualized care plans, and deliver evidence-based therapy with the autonomy to schedule visits, include telehealth when appropriate, and collaborate with an interdisciplinary team to promote mobility and aging in place.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
