FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Milford, DE, in home or senior living settings. You’ll deliver evaluations, develop personalized plans, and administer evidence-based therapy while leveraging telehealth for flexible scheduling.

You’ll work with an interdisciplinary team, educate patients and caregivers, and advance aging in place through a patient-centered Geriatric House Calls model.

Formación

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document in FOX’s EMR system according to regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
