Job summary

Qualifications

• Licensed Physical Therapist in the state(s) of practice or eligibility to obtain in Delaware.
• DPT or equivalent degree from an accredited program.
• Proficiency with EMR systems and basic computer skills.

Responsibilities

• Provide one-on-one physical therapy to older adults in home, senior living communities, or other settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility and independence.
• Document in the EMR per regulatory standards.
• Collaborate with an interdisciplinary team.
• Educate patients and caregivers on safety and aging in place.
• Offer telehealth/virtual care when appropriate.
