Job summary

HealthYes Staffing Network is seeking an experienced Geriatric Nurse Practitioner for facility-based care across Broward and Palm Beach counties in South Florida. The role covers ALF, ILF, and SNF settings with physician support and a flexible daily schedule.

You will provide primary, preventive, and acute care to adult and geriatric patients, with a productivity target of about 70 encounters per week and routine documentation in eClinicalWorks.

Qualifications

• Active Florida APRN license and board certification.
• Current DEA registration required.
• Geriatrics or internal medicine experience preferred.
• Strong clinical assessment and diagnostic skills; good communication.
• Willingness to travel among assigned facilities; autonomous practice.

Responsibilities

• Provide primary, preventive, and acute care to adult and geriatric patients.
• Conduct comprehensive assessments and follow-up visits.
• Evaluate changes in condition and acute medical concerns.
• Diagnose and manage hypertension, diabetes, dementia, and other chronic conditions.
• Prescribe medications within Florida regulations and manage therapy.
• Complete clinical orders and individualized care plans.
• Coordinate care with physicians, facility teams, specialists, patients and families.
• Participate in interdisciplinary care planning where appropriate.
• Address HEDIS, quality measures, and risk-adjustment requirements.
• Maintain complete and timely documentation in eClinicalWorks.
• Independently manage assigned patients and facility responsibilities.
