Job summary

HealthYes Staffing Network is seeking an experienced Geriatric Nurse Practitioner for facility-based care across Broward and Palm Beach counties in South Florida. The role covers ALF, ILF, and SNF settings with physician support and a flexible daily schedule.

You will provide primary, preventive, and acute care to adult and geriatric patients, with a productivity target of about 70 encounters per week and routine documentation in eClinicalWorks.

Qualifications

• Active Florida APRN license and board certification.
• Current DEA registration required.
• Geriatrics or internal medicine experience preferred.
• Strong clinical assessment and diagnostic skills; good communication.
• Willingness to travel among assigned facilities; autonomous practice.

Responsibilities

• Provide primary, preventive, and acute care to adult and geriatric patients.
• Conduct comprehensive assessments and follow-up visits.
• Evaluate changes in condition and acute medical concerns.
• Diagnose and manage hypertension, diabetes, dementia, and other chronic conditions.
• Prescribe medications within Florida regulations and manage therapy.
• Complete clinical orders and individualized care plans.
• Coordinate care with physicians, facility teams, specialists, patients and families.
• Participate in interdisciplinary care planning where appropriate.
• Address HEDIS, quality measures, and risk-adjustment requirements.
• Maintain complete and timely documentation in eClinicalWorks.
• Independently manage assigned patients and facility responsibilities.

Location: South Florida – Broward and Palm Beach Counties
Schedule: Full-Time, Flexible Facility-Based Schedule
Compensation: Approximately $140,000 - $160,000 annually at expected productivity

HealthYes Staffing Network is recruiting an experienced Nurse Practitioner for a growing Internal Medicine practice providing primary and preventive care to adult and geriatric patients throughout South Florida.

This is a facility-based opportunity covering Assisted Living Facilities, Independent Living Facilities, and Skilled Nursing Facilities across Broward and Palm Beach counties. Providers receive substantial control over their daily schedules and independently manage an established patient population with physician support available through the practice.

The productivity expectation is approximately 70 patient encounters per week, averaging about 12–15 patients per day. Workdays are structured around patient and facility responsibilities rather than remaining onsite for predetermined office hours.

• Active DEA registration required
• Prior clinical practice experience required
• Geriatrics, Internal Medicine, post-acute care, skilled nursing, or long-term-care experience preferred
• Strong clinical assessment and diagnostic skills
• Comfortable practicing with substantial autonomy
• Reliable transportation and willingness to travel among assigned facilities
• Medicare credentialing preferred
• Strong communication, organization, and care-coordination skills
• Ability and willingness to obtain and maintain personal professional liability/malpractice insurance

Compensation and Employment Terms

Annual compensation is approximately $140,000 - $160,000 when meeting the expected productivity level of approximately 12-15 patient encounters per day.

The selected provider will be responsible for obtaining, maintaining, and paying for their own professional liability/malpractice insurance at the coverage limits required by the practice and applicable facilities.
