Colonial Healthcare, part of Optum, is seeking a Board-Certified Family Medicine Physician to join our primary care team in Columbia, South Carolina. This physician-led environment offers clinical autonomy with strong operational support and access to resources of a national healthcare organization.

You will provide outpatient primary care, develop treatment plans, educate patients, and coordinate care with specialists while maintaining quality documentation in a modern EMR system

Qualifications

• Board Certification in Family Medicine.
• Active South Carolina medical license or ability to obtain.
• Current DEA certification.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide comprehensive outpatient primary care services.
• Diagnose and treat acute and chronic medical conditions.
• Develop individualized treatment plans.
• Educate patients and families on health and wellness.
• Coordinate care with specialists and ancillary services.
• Maintain high-quality clinical documentation and care standards.
