Job summary

Vinfen seeks an Autism Support Broker to work closely with families, building teams, coordinating service providers, researching funding, and supporting self-directed networks. You will ensure compliance with DDS standards and maintain data in consumer databases while facilitating communication across disciplines.

Primary duties include monthly home visits, quarterly reviews, budget support, and neutral selection of service providers.

Qualifications

• Three years in progressively responsible human services with autism experience.
• Experience working with families facing poverty and related social challenges.
• Ability to work in a professional, confidential capacity.

Responsibilities

• Meet families at home monthly and schedule quarterly reviews.
• Coordinate with DDS staff to review cases.
• Develop and implement Autism Plan of Care and maintain data in DDS systems.
• Assist families in recruiting and managing support workers.
