Qualifications

• Board-certified physician with primary care and LTC experience.
• Strong leadership and mentorship abilities for NPs/PAs.
• Ability to perform rounds and coordinate multi-disciplinary teams.

Responsibilities

• Act as the primary care provider for residents in Skilled Nursing Facilities (SNFs).
• Lead rounds with NPs and PAs; collaborate with care team to manage resident care.
• Develop treatment plans and discuss prognosis with residents and families.
• Maintain accurate medical records and ensure adherence to healthcare regulations.
• Oversee management of acute illnesses and chronic conditions in residents.
