Job summary

Jobtailor is seeking a seasoned physician to lead geriatric care initiatives in post-acute and long-term care settings. The role involves providing primary care at Fayetteville and Beckley, WV clinics and skilled nursing facilities, coordinating medical services, and partnering with NP education.

The physician will implement compliant policies and leverage new technologies to improve patient outcomes. ABMS board-certified physicians with MD/DO and Ohio license are preferred; 5+ years in

Qualifications

  • MD or DO degree required.
  • Gerontology certification preferred.
  • MD or DO license in State of Ohio.
  • ABMS Board Certified in Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative.
  • 5+ years in geriatric or long-term care population.

Responsibilities

  • Provide primary care for residents in skilled nursing facilities and clinics.
  • Coordinate medical care, including emergency treatment procedures.
  • Maintain liaison with attending physicians.
  • Implement methods to maintain quality of care under surveillance.
  • Help reduce hospital readmissions through a proactive care model.

Job description

  • Assume the administrative authority, responsibility, and accountability of implementing medical services, policies, and procedures according to federal, state and local regulations and current post-acute and long-term care medicine practices
  • Coordinate medical care, including emergency treatment procedures
  • Maintain effective liaison with attending physicians
  • Implement methods to keep the quality of care under constant surveillance
  • Provide necessary physician services for residents within skilled nursing facilities and/or clinic environment
  • Provide primary care at the Fayetteville and Beckley, West Virginia practices
  • Help reduce patients' return to hospital and achieve better outcomes through a care model combining bedside manner and innovative technologies
Requirements
  • M.D. or D.O. required
  • Gerontology certification preferred
  • MD or DO license in State of Ohio
  • ABMS Board Certified in: Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative
  • 5+ years of experience working with geriatric or long-term care population
  • Demonstrated experience working with and educating NP’s
  • Experience in LTC setting preferred
  • Familiarity with CMS, Medicare Advantage, SNP Rules and Regulations
  • HCC and Risk Adjusted reimbursement methodology
  • Inpatient/SNF utilization metrics and bed day management
  • State specific collaborative practice agreements and midlevel practitioner licensing
  • Knowledge of EMR
