Job summary

Eventus WholeHealth seeks an Attending Physician to lead population-health oriented care across assigned facilities in their Outcomes-Centered Healthcare (OCH) model. You will collaborate with Advanced Practice Clinicians and facility leadership to ensure high-quality, team-based care and professional client relations.

The role may include designation as Medical Director, providing clinical leadership and oversight within one or more facilities.

Qualifications

• MD/DO required; board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• CMD certification a plus; required within 2 years if serving in a Medical Directorship capacity
• Active and unrestricted medical license in applicable state(s); DEA registration as required
• Minimum 2-3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred
• Experience collaborating with interdisciplinary teams and facility leadership
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow

Responsibilities

• Lead population-health driven care across assigned facilities in Eventus WholeHealth's OCH model.
• Provide clinical leadership in Medical Directorship capacity when designated.
• Collaborate with Advanced Practice Clinicians and facility leadership to ensure high-quality, team-based care.
