Job summary

Eventus WholeHealth seeks an Attending Physician to lead population health within the Outcomes-Centered Healthcare model. You will provide clinical leadership in client facilities and work closely with Advanced Practice Clinicians to deliver high-quality, team-based care across assigned sites.

The role may include Medical Directorship responsibilities, offering professional growth and the opportunity to influence resident care and facility operations in post-acute and value-based care settings.

Qualifications

• MD/DO required; board cert in IM/FM/Geriatrics preferred
• Active and unrestricted medical license in applicable state(s) and required DEA registration
• CMD certification a plus; required within 2 years for Medical Directorship capacity
• 2–3 years clinical experience in post-acute, long-term care, or value-based care settings preferred
• Experience collaborating with interdisciplinary teams and facility leadership
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow

Responsibilities

• Lead population health through Eventus WholeHealth's Outcomes-Centered Healthcare model
• Provide clinical leadership in client facilities and collaborate with Advanced Practice Clinicians
• Ensure high-quality, accountable, team-based care across assigned facilities
• May serve in Medical Directorship capacity with clinical leadership and oversight
