Job summary

Eventus WholeHealth in the United States is seeking an Attending Physician to lead population health initiatives within post-acute and long-term care facilities. You will collaborate with Advanced Practice Clinicians and facility leaders to deliver high-quality, teamwork-driven care under our Outcomes-Centered Healthcare model.

You will have clinical leadership responsibilities, potential Medical Directorship oversight, and a commitment to evidence-based care, with a focus on improving resident

Qualifications

• MD or DO required; board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• CMD certification is a plus; required within 2 years if serving in Medical Directorship.
• Active and unrestricted medical license in applicable state(s); DEA registration as required.
• Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred.
• Experience collaborating with interdisciplinary teams and facility leadership.
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow.

Responsibilities

• Provide clinical leadership and accountability for high-quality, team-based care across assigned facilities.
• Lead population health initiatives within Eventus WholeHealth’s OCH model in client facilities.
• Collaborate with Advanced Practice Clinicians to align care plans and optimize resident outcomes.
• Support potential Medical Directorship governance and oversight as needed.
