APM in Philadelphia seeks an Attendance Case Manager for Community Schools to provide case management and outreach to students and families, coordinating with School District staff to improve attendance and academic outcomes.

The role requires a Bachelor's degree in social work or related field, at least 1 year in social work with case management, and the ability to work evenings/weekends as needed. Benefits include comprehensive health plans, retirement, and paid time off.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Minimum of 1 year of social work experience that includes case management and work with children, youth, and families.
• Experience in community services and collaboration with schools is preferred.

Responsabilités

• Provide services to a caseload of children, youth, and family members with documented case management activities.
• Document all activities in case files and databases.
• Conduct home visits and family outreach to support attendance and student well‑being.
• Coordinate referrals to external services and ensure linkages are secured.
• Participate in case conferences, team meetings, and school partnerships to support student movement.
