Job summary

APM CUA in Philadelphia, PA, is seeking a compassionate Case Aide to support the CUA CM and Supervisor in delivering the SINGLE Case Plan. You will conduct home, school, and community visits, help access resources, supervise visits, and provide transportation support for families and children as needed.

You will document activities in ECMS within six business days, coordinate with the CM, and participate in Family Team Conferences and court hearings when required.

Qualifications

• High School diploma or GED with two years of experience working with children and families.
• Proficiency in English and Spanish preferred.
• Strong organizational skills.
• Demonstrated written and oral communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Work with the CUA Support Team toward achieving the SINGLE Case Plan objectives.
• Provide home, school, and community visits and assist in accessing resources.
• Supervise visits and report safety concerns to Case Manager.
• Provide transportation support for children and family to appointments and activities.
• Upload documents into ECMS and monitor visitation schedules.
