Job summary

ADP is seeking a Case Manager in Philadelphia, PA to support at-risk youth through intensive programming in the Aftercare Evening Resource Center (AERC) setting. The role focuses on afternoon and evening hours (12pm-8pm or 1pm-9pm), Mon-Fri, to maximize family involvement and client engagement.

The Case Manager will connect families with services, help document academic progress, and collaborate with schools and counselors to develop post-secondary plans, while ensuring compliance with program

Qualifications

• Bachelor's degree in human services (BSW) or counseling with two years' family-based social work experience.
• Act 168, FBI, child abuse and PA clearances within one year of hire.
• Ability to work in a team and navigate educational and child welfare systems.

Responsibilities

• Resolve issues and promote positive youth development and academics.
• Provide service linkages for family members as appropriate.
• Collaborate with school counselors to monitor academic progress.
• Attend monthly team meetings and coordinate with DHS and court personnel.
