The Pavilion at Stow is a skilled nursing and rehabilitation center in Stow, OH, seeking a Licensed Practical Nurse (LPN) for a full-time role. You will provide direct nursing care to residents and supervise certified nursing assistants in compliance with regulations and facility policies.

You will administer medications, perform wound care, and coordinate admissions and transfers while collaborating with physicians to ensure quality resident care.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• Current CPR certification.
• IV certification, if applicable.
• Prior experience in long-term care preferred but not required.

Responsabilités

• Promotes holistic care using the nursing model.
• Ensures policy compliance by nursing staff.
• Assists with admissions, discharges and transfers as required.
• Monitors resident status, notifies physicians and families as needed.
• Transcribes physician orders; administers medications and observes adverse effects.
• Performs wound care and documents changes.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership to the unit.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.

• Follows the nursing model to promote holistic care for the residents.

• Ensures that policies and procedures are complied with by nursing personnel assigned.

• Participates in the admission, discharge and transfers of residents as required.

• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.

• Transcribes physician orders to medical record and carries out orders as written.

• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.

• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.

• Schedules follow up appointments for residents and transportation needs as indicated.

• Provides nursing leadership to nursing personnel assigned to the unit.

• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.

• A Nursing degree from an accredited college or university.

• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.

• Current CPR certification.

• IV certification, if applicable
