Job summary

The Pavilion at Stow is seeking a licensed practical nurse (LPN) to deliver high-quality nursing care in a skilled nursing and rehabilitation setting. The LPN will supervise certified nursing assistants, administer medications, and coordinate resident care in compliance with federal, state, and local regulations.

The ideal candidate holds an active LPN license, CPR certification, and may have IV certification. This full-time position offers a competitive wages and benefits package in Stow, OH.

Qualifications

Responsibilities

• Directs daily activities of the certified nursing assistants in accordance with facility policies and guidelines.
• Promotes holistic care following the nursing model.
• Ensures policy compliance by nursing staff.
• Assists in admission, discharge and transfers of residents.
• Assesses changes in residents’ status and notifies physician and family as needed.
• Transcribes physician orders and carries out orders as written.
• Prepares and administers medications per orders; observes for adverse effects.
• Performs wound treatments and documents changes.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership to unit staff.
• Other duties as assigned.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.
• Schedules follow up appointments for residents and transportation needs as indicated.
• Provides nursing leadership to nursing personnel assigned to the unit.
• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable
