Civitas Senior Living in Colorado Springs, CO, invites an experienced professional to join as Assistant Wellness Director (SCFH). You will report to the Wellness Director and support residents’ daily living activities while promoting independence.

The role emphasizes staffing coordination, wellness assessments, and seamless care coordination with vendors. A strong background in senior care and medication management is preferred, with flexibility to work extended hours, weekends, and holidays.

Qualifications

• Background in senior care, preferably in a wellness or medication management capacity.
• Ability to manage multiple tasks and deliver results within necessary time frames.
• Willingness to work extended hours, weekends, and holidays as the role demands.
• Detail-oriented with proficiency in various computer programs.

Responsibilities

• Coordinate staffing and scheduling using modern staffing software; assist with recruitment and training to ensure a robust healthcare team.
• Complete comprehensive assessments and individualized care plans, emphasizing personalized wellness.
• Liaise with internal staff and external third-party vendors to ensure seamless care coordination.
• Lead monthly staff meetings and daily team huddles, cultivating an environment of continuous learning and collaboration.
• Manage service agreements, update physician orders, and maintain Electronic Medication Administration Records (EMAR).
• Perform weekly cart audits, MAR reviews, and expired medication retrieval.
• Assist with incident reports and investigations, ensuring adherence to best practices in senior care.
• Other duties as assigned.
