OhioHealth Neuro Transitional Center in Dublin, OH, seeks an Assistant Residential Manager - RN to lead clinical and administrative operations for our Neuro Transitional Center. The role focuses on patient safety, quality of care, and staff supervision within a home-like inpatient environment.

The RN will oversee multidisciplinary staff, ensure regulatory compliance, and coordinate with physicians and nurses to deliver comprehensive rehabilitation services.

Qualifications

• Current RN licensure in good standing.
• BLS certification through the American Heart Association.
• Strong leadership and regulatory compliance awareness.
• Experience with long-term/post-acute care settings preferred.

Responsibilities

• Oversee and coordinate medical rehabilitation services in line with center policies and treatment model.
• Supervise Rehabilitation Coaches and provide leadership to maintain high standards of care.
• Manage residential operations with administrative and clinical responsibilities.
• Ensure facility compliance with HHSC ALF Licensure and other regulatory requirements.
• Coordinate communication among treatment team, physicians, and on-call staff.
• Develop and track patient care goals within the plan of care and ensure timely documentation.
• Oversee medication management processes, MAR audits, and input orders into PCC/EHR.
• Provide patient and family education regarding diagnosis and required care.
• Participate in on-call rotations and required meetings.
• Transport patients for community outings when needed.

Under the supervision of the Residential Manager, the Assistant Residential Manager oversees and coordinates medical rehabilitation services in alignment with the Neuro Transitional Center’s policies, procedures, and treatment model. Ensures patient safety and fosters a positive patient experience throughout the continuum of care. Supervises Neuro Rehabilitation Coach Supervisors and Neuro Rehabilitation Coaches, providing leadership and support to maintain high standards of care and staff performance. Collaborates with the Rehab Residential Manager to manage residential operations, including both administrative and clinical responsibilities, while contributing to a safe,therapeutic, and well-organized environment.

• Assists the Residential Manager with ensuring that the residential facility operates in compliance with HHSC ALF Licensure, CARF and other regulatory/accreditation organizations, including assisting with creating procedures to ensure adherence.
• Manages documentation ensuring completeness, timeliness and accuracy in accordance with specialty area
• Communicates with On Call and/or physicians/office nurse, as needed.
• Ensures communication of pertinent information including specific concerns regarding patient safety or health to the treatment team in a timely manner. Ensure residential communication are up to date.
• Assists in the development and tracking of transdisciplinary goals within the patient plan of care.
• Assists the Residential Manager with the oversight of clinical and regulatory compliance, orientation, training, supervision, and ongoing education and competency assessments of residential direct care staff across the delivery sites and in the community. Key treatment model components include: supervision and assistant levels, environmental modifications, communication, treatment documentation, following patient daily activity plans and overall treatment plan goals, shift reporting.
• Participates in on-call rotation to provide support to staff after hours and on weekends.
• Participates in required meetings which may include patient staffing, family conferences, admissions, EOC, residential meetings, etc.
• Occasionally transports patients on community outings
• Training and oversight of nurse delegation
• Conducts weekly MAR audits, verification of medication administration records and input of orders into EHR. Conducts head to toe assessment of new admits, and ongoing required assessments. Completes residential intake and ensures all admission orders are entered into PCC.

Minimum Qualifications

• Current licensure as a Registered Nurse (RN) required.
• BLS certification through the American Heart Association is required.
• Current State Driver's License in good standing required.
• Nursing Facility Administrator License as defined by state requirements, required within 90 days or as defined by regulation.

Preferred Qualifications

• Two years of leadership experience in a post-acute care setting preferred.
