Evolve Therapy Services, LLC is seeking an Assistant Director of Rehabilitation (PTA/OT/PTSLP) for an in-house Skilled Nursing Facility located in Hopkins, MN. The role supports the Director of Rehabilitation in managing therapy operations and ensuring high-quality patient care.

The candidate will oversee the therapy program, ensure regulatory compliance, and train staff to meet policy standards. Strong leadership, communication, and problem-solving skills are essential for success in this role.

Qualifications

• Leadership and team supervision experience in therapy programs.
• Strong knowledge of clinical standards and regulatory requirements.
• Proven ability to partner with interdisciplinary teams and DORs.

Responsibilities

• Assist the Director of Rehabilitation with therapy operations at the facility.
• Oversee all aspects of the therapy program in assigned facilities.
• Ensure adherence to regulatory requirements, policies, and documentation standards.
• Supervise therapy staff, enforce productivity standards, and promote quality care.
• Train staff on procedures and collaborate with interdisciplinary teams.
