Job summary

Sheppard Pratt is hiring for a Community Program - Unlicensed, Full-Time, Day shift, in-person position. The role leads the Segue Residential Transition Program, guiding clients from state-run hospitals to community living and coordinating care with a multidisciplinary team.

You will supervise staff, oversee in-reach processes, manage budgets and quality systems, and ensure compliance with licensing and funding standards. A CPRP cert is required or willingness to obtain.

Qualifications

• A high school diploma or GED with 5 years of experience.
• An associate degree with 4 years of experience.
• A bachelor’s degree with 2 years of experience.
• Driver’s license with 3-points or less and access to an insured vehicle during work hours.
• Candidate must possess CPRP certification by start date or be willing to obtain it.

Responsibilities

• Oversee program development, coordinate care, and supervise staff with focus on day-to-day operations of in-reach and service coordinator teams.
• Oversee all in-reach processes, ensure services are authorized and compliant with guidelines.
• Supervise staff and ensure onboarding, training, and development.
• Develop and manage the program budget, policies, procedures, and quality improvement systems.
• Maintain licensing, accrediting, and funding standards; implement corrective action plans.
• Conduct clinical assessments, develop treatment plans, and implement interventions for individuals with serious mental illness.
• Support clients involved in the forensic system; ensure compliance with conditional releases and collaboration with mental health courts.
• Develop strategies to help individuals transition from long-term hospital stays to community living.
