Qualifications

• Master's Degree in Social Work or related field.
• Minimum one year of experience in a HealthHome program and/or Care Coordination program.
• Minimum three years supervisory experience in a behavioral health setting.
• General understanding of chronic conditions (HIV, Substance Use, Mental Illness).
• Flexibility to respond to after-hours needs and schedule changes.

Responsibilities

• Coordinates and implements case management services for an assigned treatment team.
• Helps implement policies, procedures, and systems for effective service delivery.
• Provides regular administrative supervision to assigned staff.
• Documents supervisory sessions with staff.
• Identifies staff training needs and plans/conducts training to improve competencies.
• Ensures accurate, complete, timely client treatment records comply with regulations.
• Upholds confidentiality per NYS and agency policy (42 CFR, HIPAA).
• Networks with other departments as a management team member.
• Maintains compliance with the Code of Conduct and Ethics.
