You will supervise clinicians, care managers, a nurse, and a peer specialist while coordinating with partner agencies. This role emphasizes outreach, housing placement, discharge planning, and advocacy to address disparities in access to care.

Qualifications

  • Licensed Master’s degree in social work, mental health counseling, nursing, or psychology.
  • Experience with homeless populations is preferred but not required.
  • Knowledge of counseling principles for mental illness and substance use disorders.
  • Comfort with field-based work and coordinating with multiple partners.

Responsibilities

  • Provide clinical and operational leadership to the SOS team with the Program Director.
  • Conduct outreach at transit hot spots and during hospital admissions.
  • Collaborate with street outreach teams, police, hospitals, MTA, and DHS.
  • Assess health and social needs using formal risk tools.
  • Coordinate housing applications and moving-in processes with SOR Hub.
  • Participate in hospital discharge planning and connect to community resources.
  • Offer short-term counseling pre- and post-housing placement.
  • Collect data and report outcomes to inform care strategies.
