Job summary

ACMH INC in New York, NY is seeking a Case Manager for Safe Options Support (SOS). Qualifications

• Bachelor’s degree or higher in psychology, social work, sociology or related field or NY State LPN license.
• Four years of past work case management experience may be considered in lieu of Bachelor’s degree.
• Experience with homeless populations preferred but not required.
• Computer proficiency and good documentation skills.

Responsibilities

• Persistent outreach and engagement in transit system hang-outs or hospital settings.
• Assess health and social needs using SOS tools and formal risk assessments.
• Collaborate with SOR Hub to identify housing, assist with applications and move-in.
• Participate in hospital discharge planning to locate community resources.
• Collects and reports data; work with SOS teams to inform care delivery.
• Assist participants with housing-related issues after securing housing.
• Build relationships with community providers to connect services.
• Navigate appointments with participants; provide travel training and support.
• Review documentation and conduct psychosocial assessments of medical, housing, and social needs.
• Gather collateral information to support health and psychosocial needs.
• Monitor progress toward care plan goals; participate in team meetings.
• Perform other related duties as assigned.
