The Pavilion at Jacksonville is seeking an Assistant Director of Nursing / MDS Coordinator to join our nursing leadership team at a 60-bed skilled nursing and rehabilitation center on Edgewood Avenue in Jacksonville.

This combined ADON/MDS Coordinator role requires hands-on MDS experience, Florida RN licensure, and prior SNF/LTC leadership. You’ll work with a close-knit staff and benefit from direct access to ownership and a responsive nursing leadership team.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in Florida.
• Prior experience as an Assistant Director of Nursing (ADON) or MDS Coordinator.
• Hands-on MDS (Minimum Data Set) experience.
• Skilled nursing facility (SNF) / long-term care (LTC) experience is required.

Responsibilities

• Support the Director of Nursing in day-to-day clinical operations, staff supervision, and quality of care.
• Own the MDS process: complete accurate, timely assessments and manage the RAI schedule.
• Coordinate care plans with the interdisciplinary team and lead care plan meetings.
• Monitor documentation and clinical practice for compliance with state and federal regulations.
• Step in as clinical leader when the DON is unavailable.

This is a combined Assistant Director of Nursing / MDS Coordinator role at a 60-bed skilled nursing and rehabilitation center on Edgewood Avenue in Jacksonville, where the building is small enough that you know every resident and every nurse by name, and the Director of Nursing (DON) is right down the hall.

We're looking for an Assistant Director of Nursing / MDS Coordinator who has done this work before. Prior skilled nursing facility (SNF) / long-term care (LTC) experience is required, and hands-on MDS (Minimum Data Set) experience is a must.
