The Johns Hopkins Hospital in Baltimore, MD is seeking an Assistant Director of Transitions to oversee the Readmissions Department, drive staff management, program development, and strategic planning to improve care transitions from inpatient to community settings.

This leadership role supervises home care coordinators, discharge planners, and transition guides, collaborates with affiliates and facilities, and aims to reduce length of stay, prevent readmissions, and enhance the patient

Qualifications

• Minimum of 5 years’ progressive leadership experience within a health system.
• Two years’ nurse management experience on an acute care inpatient unit is highly preferred.
• Healthcare professional licensure is preferred.

Responsabilités

• Oversee day-to-day operations of the Readmissions Department, including staff management, program development, and strategic planning.
• Lead efforts to improve care transitions for patients moving from inpatient care back to the community.
• Supervise home care coordinators, discharge planners, liaisons, and transition guides, while partnering with affiliates to improve access, patient acceptance, safe care, and efficiency.
• Establish collaborative relationships with nursing facilities and assisted living facilities and develop strategies to build community capacity to support patient discharges.
• Play a key role in reducing length of stay, preventing readmissions, and enhancing the patient experience.
