Job summary

Triangle Springs, part of Lifepoint Health, seeks an Assessment Supervisor to oversee responses to services, initial evaluation, and care level determinations while coordinating with physicians and staff in our Raleigh, NC facility. This full-time, 4pm–12am shift emphasizes prompt, compassionate patient care and efficient admission processes.

You will coach staff, ensure proper documentation, manage shift staffing, and minimize overtime.

Qualifications

• Associates in nursing or Master's in social work/counseling are required.
• Current unencumbered RN or social work license is required.
• CPR certification and CPI within 30 days.

Responsibilities

• Facilitates response to requests for services, including initial response, assessment, referral, and appropriate follow-up.
• Conducts initial evaluation and documents patient symptoms, functioning, and needs for LOC determination.
• Maintains excellent customer service for patients, physicians, visitors, and coworkers.
• Ensures staff documentation, call logging, and insurance communication are completed for each shift.
• Educates and coaches staff and acts as a clinical decisions point person.
