Job summary

Triangle Springs is seeking an Assessment Specialist for outpatient care in Raleigh, NC. You will participate in the full assessment and referral process, documenting symptoms, needs, and LOC determinations, while ensuring timely, courteous service to patients and staff.

The role requires a nursing degree or state license, plus CPR and CPI training within 30 days. Prior psychiatric facility experience is preferred, with strong emphasis on collaboration with physicians and insurance providers.

Qualifications

• Graduate from an accredited program of professional nursing or relevant state licensure required.
• Experience in a psychiatric health care facility preferred, with involvement in chemical dependency, dual diagnosis, and psychiatric patients.
• Experience in patient assessments, family motivations, treatment planning and communication with external review organizations or comparable entities.
• Current unencumbered license to practice by the State Board of Nursing.
• CPR certification and Crisis Prevention Training (CPI) within 30 days.

How you'll contribute

AnAssessment Specialist who excels in this role:

• Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity.
• Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination.
• Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner.
• Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA.
• Maintains all preadmission and admission paperwork with accuracy and completes all precertification with insurance companies.
• Other responsibilities as assigned by leadership

What we're looking for

• Education: Graduate from an accredited program of professional nursing or relevant state licensure required
