Job summary

Triangle Springs is part of Lifepoint Health and seeks an Assessment Specialist to support behavioral health services in the Raleigh area. The role emphasizes timely assessment, care coordination, and ensuring proper admission and follow-up for patients in need of services.

Ideal candidates hold a nursing license or relevant licensure, with experience in psychiatric settings, patient assessments, and collaboration with external review entities.

Qualifications

• Graduate from an accredited program of professional nursing or relevant state licensure.
• Experience in a psychiatric health care facility with chemical dependency or dual diagnosis work.
• Experience in patient assessments, treatment planning, and communicating with external review entities.
• Current unencumbered license to practice by the State Board of Nursing.
• CPR certification and CPI training within 30 days.

Responsibilities

• Facilitates response to requests for services: initial response, assessment, referral, level of care, admission, and follow-up.
• Conducts initial evaluation, initiates the assessment, documents symptoms, functioning, and needs; informs MD for LOC determination.
• Demonstrates excellent customer service with patients, physicians, visitors, and coworkers.
• Maintains knowledge of Emergency Detention paperwork and EMTALA requirements.
• Keeps preadmission and admission paperwork accurate and completes precertification with insurers.
• Other responsibilities as assigned by leadership.

How You'll Contribute

• Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity.
• Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination.
• Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner.
• Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA.
• Maintains all preadmission and admission paperwork with accuracy and completes all precertification with insurance companies.
• Other responsibilities as assigned by leadership

What We're Looking For

• Education: Graduate from an accredited program of professional nursing or relevant state licensure required
• Experience: Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, and psychiatric patients preferred.
• Experience in patient assessments, family motivations, treatment planning and communication with external review organizations or comparable entities
• License: Current unencumbered license to practice by the State Board of Nursing
• Additional Requirements: CPR certification and Crisis Prevention Training (CPI) within 30 days.
