Assessment Specialist - Flex Pool - Clark County

We are currently seeking a Community Health Worker in the Clinical department to conduct program assessments using established tools and processes. This home-based position requires independent management of a caseload as needed.

Position Details

Flex staff are employed on an "as needed" basis. When services are required by CareStar, Inc., employees may work up to a maximum of 29 hours per week. These employees are generally not eligible for benefits unless specified in the Employee Benefit Eligibility Policy but are eligible for mileage reimbursement according to CareStar policy.

Responsibilities

• Conduct program assessments using approved tools and processes.
• Manage a caseload independently in a home-based setting.
• Adhere to CareStar policies and procedures.
• Maintain confidentiality of patient information and comply with all privacy regulations.
• Follow the Acceptable Use Policy for all information systems.

Qualifications

• Active license as RN, LSW, or LISW preferred.
• At least 12 months of experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs is desirable.
• Ability to adhere to professional and organizational standards.

Location

This position is based in Clark County and involves coverage of surrounding counties.
