Assessment Specialist - Flex Pool - Stark and surrounding counties

CareStar, Inc. is currently seeking an Assessment Specialist in the Clinical department to conduct program assessments using established tools and processes. This home-based position involves managing a caseload independently as needed.

Flex staff are employed on an "as needed" basis. When services are required by CareStar, Inc., employees may work up to a maximum of 29 hours per week. These employees are not eligible for benefits unless otherwise specified in the Employee Benefit Eligibility Policy but are eligible for mileage reimbursement according to CareStar policy.

THE POSITION

CareStar is looking for motivated individuals who:

• Hold an active license as RN, LSW, or LISW (preferred).
• Have 12 months of experience in home and community-based services within the past 5 years.
• Possess knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable).
• Adhere to the CareStar Rules in performing job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality related to patient information. Unlawful disclosures outside the proper course of duty will be treated as a serious disciplinary offense. Follow the Acceptable Use Policy when using CareStar's information systems.

THE LOCATION

This position is open in Lawrence County and is responsible for covering the surrounding counties.
