Job description

Assessment Specialist - Flex Pool - Summit and surrounding counties

CareStar, Inc. is currently seeking an Assessment Specialist in the Clinical department to conduct program assessments using established tools and processes. This is a home-based position that works independently to manage a caseload as needed.

Position Details

This is a flex staff position, employed on an "as needed" basis. When services are required by CareStar, Inc., the employee may work up to a maximum of 29 hours per week. These employees are not eligible for benefits unless otherwise specified in the Employee Benefit Eligibility Policy, but are eligible for mileage reimbursement according to CareStar policy.

Responsibilities

  • Conduct program assessments using approved tools and processes.
  • Manage a caseload independently.
  • Maintain confidentiality of patient information and adhere to all relevant policies and procedures.

Qualifications

  • Active license as RN, LSW, or LISW is preferred.
  • At least 12 months of experience in home and community-based services within the past 5 years.
  • Knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs is desirable.
  • Adherence to CareStar's rules, policies, and procedures.
  • Ability to maintain confidentiality and follow information security policies.

Location

This position is based in Lawrence County and covers surrounding counties.
