Job summary

Triangle Springs, part of Lifepoint Health, seeks an Assessment Specialist to support a vital mission of healthier communities in Raleigh, NC. The role emphasizes assessment, care coordination, and timely communication with patients, families, and care teams.

Responsibilities include evaluating patients, facilitating referrals, and ensuring accurate documentation and compliance with insurance and regulatory requirements. Full-time schedule from 7am to 7:30pm is offered.

Qualifications

  • Education: Associate's Degree in Nursing or Master’s degree in Social Work/Counseling.
  • Licensing: Current clinical, social work, or RN license as required by state regulations.
  • Experience: Psychiatric health care setting experience preferred.

Responsibilities

  • Facilitates response to requests for services, including assessment, designation of level of care, admission, and follow-up.
  • Conducts initial evaluation and documents patient symptoms, functioning, and needs for LOC determination.
  • Maintains patient communications with clarity and professionalism, supporting physicians, patients, and staff.
  • Manages preadmission/admission paperwork and precertification with insurers.
  • Performs other duties as assigned by leadership.

How You\'ll Contribute
  • Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity.
  • Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination.
  • Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner.
  • Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA.
  • Maintains all preadmission and admission paperwork with accuracy and completes all precertification with insurance companies.
  • Other responsibilities as assigned by leadership

What We\'re Looking For
  • Education: Associate\'s Degree in Nursing Required or Master\'s degree in Social Work/Counseling required.
  • Experience: Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, and psychiatric patients preferred.
  • License: Current clinical, social work, or RN license as required by state regulations.
  • Additional Requirements: CPR certification and Crisis Prevention Training (CPI) within 30 days.
