This role involves evaluating eligibility, creating initial care plans, and preparing families for ongoing support, including orientation to the Careforth platform. All assessments are completed within set timelines for prompt service activation. No night shifts or weekends required, and no on-call duties! The pay range for this position is $74,300 - $90,900. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
