Job summary

Lifepoint Health's Westpark Springs is seeking an Assessment Coordinator to help route callers, schedule initial psychiatric assessments, and support admissions.

You will collaborate with the Care Coordination team, handle clerical duties, and assist patients in navigating treatment options within a mission-driven healthcare system.

Qualifications

• Education: High School Diploma or GED preferred.
• Basic Life Support certification is required within 30 days of hire.
• Experience: Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred. Must have strong clinical assessment skills.

Responsibilities

• Assess walk-in and phone callers to route to services and schedule initial psychiatric assessments.
• Answer inquiry calls about treatment, present options, document, and set appointments.
• Collaborate with business office to ensure smooth admission; triage to other sources.
• Provide clerical support to Care Coordination Manager; data entry.
• Maintain stock of forms and supplies for Care Coordination.
• Check in patient belongings; process consent forms; coordinate admission paperwork.
• Schedule assessments; orient patients to units/outpatient programs; assist with discharges; send docs to referrals.
• Perform other duties as assigned.

How you'll contribute

An Assessment Coordinator who excels in this role:

• Assess walk‑in and phone callers to determine appropriate services or referrals and schedule initial psychiatric assessments for individuals seeking treatment for substance abuse, dual diagnosis, or emotional disorders.

• Answer inquiry calls regarding treatment and facility services, present treatment options, document calls, and set appointments for callers seeking treatment.

• Collaborate with business office staff to ensure an efficient admission process and refer or triage patients to other treatment sources when appropriate.

• Provide clerical support to the Care Coordination Manager and team, including data entry into the electronic record system.

• Maintain an appropriate stock of forms and supplies to support the Care Coordination Department.

• Check in patient belongings, process consent forms, and coordinate admission paperwork.

• Schedule assessment appointments, orient patients to units and outpatient programs, assist with discharges, and send documentation to referral sources within required timeframes.

• Perform other duties as assigned.
