Job summary

Sycamore Springs, part of Lifepoint Health, seeks a dedicated Care Coordination Coordinator to support walk-in and phone referrals, schedule initial psychiatric assessments, and aid admissions processes. You will maintain accurate records in the Medik system and help manage unit paperwork.

Ideal candidates have psychiatric health care experience, CPR and CPI readiness within 30 days, and strong clinical assessment skills to support patients and families through treatment decisions.

Qualifications

• CPR certification and CPI training within 30 days are required.
• Experience in a psychiatric health care facility is preferred with strong assessment skills.
• CPR and CPI completion within 30 days are essential for duties.

Responsibilities

• Provide clerical support to the Care Coordination Manager and team.
• Enter information accurately into the Medik system and maintain forms and supplies.
• Schedule initial psychiatric assessments and coordinate admissions paperwork.

How You'll Contribute

Coordinator assess the needs of walk-in and phone callers to ensure their referral to the service or resource to best address those needs. Coordinator assists in scheduling initial psychiatric assessment on patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders and presents symptoms to psychiatrist for best recommendation of care. Coordinator refers and triages patients to other sources of treatment when appropriate. Coordinator answers all inquiry calls regarding treatment and the facility, presents treatment options, documents calls and sets appointments for callers seeking treatment. Coordinator works with the business office to ensure smooth transition into the admission process.

Essential Functions

• Provides clerical support to the Care Coordination Manager and Care Coordination Team and acts as pack up to front desk reception. Enters information into the Medik system accurately.
• Maintains appropriate stock of paperwork/forms and supplies to support the Care Coordination Department.
• Checks in patient belongings with accuracy and documents appropriately.
• Processes consent forms and coordinates admission paperwork for unit.
• Provides assistance to other units and departments as needed.
• Sends appropriate documentation to referral sources in the appropriate timeframe.
• Provides assistance with discharges.
• Schedules appointments for incoming assessments paying close attention to the needs of the patient.
• Assists in the orientation of patients to units and to outpatient programs.

Qualifications And Requirements

Education: High school degree or equivalent preferred.

Experience: Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred. Must have strong clinical assessment skills.

Additional Requirements: CPR certification and Crisis Prevention Training (CPI) within 30 days.
