Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living in Gresham, OR seeks a Part-time/On-Call Support Services Provider for a Deaf and Deaf-Blind community. The role emphasizes ASL/Tactile communication, care planning, and advocacy to empower residents to age in place while maintaining independence.

Responsibilities include coordinating interpreters, facilitating access to services, and promoting accessible communication.

Qualifications

• Bachelor's degree in special education, social work, counseling or related field preferred, or equivalent knowledge of state regulations for assisted living communities.
• Exceptional fluency in American Sign Language (ASL) and familiarity with tactile sign language, Pro-Tactile, and diverse deaf-blind communication systems.
• Minimum of 1–3 years of relevant professional experience with elderly or disabled populations or in a deaf and blind community.
• State certification (SSP) and must maintain CPR/BLS during employment.
• Excellent verbal and written communication to interact with seniors, families, and staff.
• HIPAA confidentiality awareness and ability to maintain resident records.

Responsibilities

• Relay information using the resident's preferred communication mode (ASL, tactile, braille).
• Coordinate on-site sign language interpreters, captioning, and augmentative communication systems.
• Assist residents with reading mail, emails, and announcements.
• Develop, monitor, and update individualized support plans with residents and families.
• Connect residents with outside agencies and specialized counseling or training.
• Coordinate transportation and accompany residents on off-site activities.
• Educate staff on Deaf culture and accessible communication; advocate for ADA rights.
