6 Degrees Search seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare team operating within the Patient Centered Medical Home model. The ARNP will partner with physicians, PAs, and nurses to deliver high-quality care across a diverse patient population.

The role emphasizes clinical autonomy, collaborative practice, and patient education, with responsibilities spanning assessments, care planning, documentation, and performance metrics

Formación

• Experience in broad primary care clinic settings.
• Experience in minor surgical techniques is preferred.
• Strong knowledge of the nursing process: assessment, planning, implementation, evaluation.

Responsabilidades

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patient health.
• Use independent clinical judgment based on evidence-based data.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Complete fee slips with accurate diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters per year, full-time).
• Provide care coordination via daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
• Discharge planning and patient self-management education.
• Explain procedures and plans to patients before execution.
• Participate in quality management and staff education.
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
