United Future is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health care organization in Washington. This permanent, full-time role operates within the PCMH model and collaborates with physicians, PAs, ARNPs, and nursing staff to deliver high-quality care.

Key responsibilities include advanced clinical assessments, care management, patient education, and ensuring timely documentation.

Qualifikationen

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in Washington.
• Prescriptive authority from Washington State.
• Experience in broad primary care clinic setting.
• Strong knowledge of nursing process: assessment, planning, implementation, evaluation.

Aufgaben

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs and nursing staff on patient care.
• Develop teaching plans to support patient health goals.
• Use independent clinical judgment based on evidence-based data.
• Maintain chart documentation within 48 hours, 95% of the time.
• Complete fee slips with matching diagnosis and procedure codes.
• Meet productivity: minimum 3,400 encounters per year (full-time).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
• Educate patients for self-management and discharge planning.
• Explain procedures and plans to patients prior to execution.
• Participate in quality management, peer review and practice development.
• Support education of students and other health professionals.
• Maintain required annual training (HIPAA, Cultural Diversity, BBP, SH).
