Job summary

Residential Transitions, Inc. (RTI) seeks ARMHS Mental Health Practitioners to support clients in the community under individualized treatment plans. You will collaborate, listen actively, and document progress toward goals in communication, living skills, and crisis intervention.

You will report to the program manager in a full-time position, with travel within Dakota County and surrounding areas and a strong emphasis on person-centered care and cultural competence.

Qualifications

• Meet Minnesota state requirements for the role via one of the listed pathways.
• Willingness to travel within the Metro Area with a valid driver’s license.
• Pass DHS background checks and related screenings.

Responsibilities

• Provide community-based ARMHS mental health support per treatment plans.
• Collaborate and actively listen; document visits and progress.
• Record progress towards treatment goals in areas like communication and skills.
• Report to program manager with no direct reports; full-time role.
