Job summary

Sweetwater Care is seeking a licensed PTA to work on-site in the Phoenix area. Under a Physical Therapist's supervision, you will assist with patient rehabilitation after illnesses or injuries, helping residents regain mobility, balance, and independence.

The role emphasizes documentation, adherence to the care plan, and collaboration with the therapy team to deliver quality patient care. A valid PTA license in Arizona is required.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care to residents aiming to recover and rehabilitate from serious illness, injury, or medical events.
• Assist in delivering quality patient care under supervision of licensed therapists.
• Document all services to support accurate billing of therapy services.
• Ensure patient care follows the Plan of Care and organization guidelines, meeting professional standards and regulations.
• Under supervision of a Physical Therapist, identify and develop care plans and implement therapy programs to restore performance.
• Identify needs for referrals to other disciplines to address patient needs under supervision of a Physical Therapist.
• Other duties may be assigned as needed.
