Qualifications

• Active state licensure in Arizona is required.
• Experience in rehabilitation or physical therapy setting preferred.
• Proficiency in accurate documentation and record-keeping.

Responsibilities

• Consult with patients to learn about their physical condition and review medical history.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records by tracking goals.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate outcomes and modify services based on patient progress.
• Provide counseling to patients, families, and other service providers.
• Communicate with supervisor and health team about progress and plans.
• Provide comprehensive discharge summaries as required.
• Document all regulatory information in the electronic system and maintain patient records.
• Participate in trainings and conferences.
• Able to supervise PT assistants and rehab aides.
• Adhere to company policies and code of conduct.
