Qualifications

• High school diploma or equivalent required.
• Prior caregiving experience preferred; healthcare or long-term care valued.
• Ability to read, write, and communicate in English.

• Knowledge of assisted living/memory care regulations preferred.
• Ability to work collaboratively in a fast-paced, team-oriented environment.
• Flexibility to work evenings, weekends, and holidays as needed.

Responsibilities

• Provide resident care and support, including activities of daily living.
• Assist residents with bathing, grooming, dressing, toileting, and mobility.
• Help residents with meals, snacks, hydration, and feeding as needed.
• Support ambulation, positioning, and ROM activities.
• Monitor comfort and address needs; report changes promptly.
• Monitor and record vital signs, weight, and intake/output.
• Document care and observations per Mosaic policies and regulations.
• Engage residents in activities to promote well-being.

Key Responsibilities

• Resident Care & Support
• Provide compassionate, hands-on care to residents, including assistance with bathing, grooming, dressing, toileting, and mobility.
• Assist residents with meals, snacks, hydration, and feeding as needed.
• Support residents with ambulation, positioning, and range-of-motion activities.
• Monitor residents’ comfort levels and promptly address needs and concerns.
• Health Monitoring & Documentation
• Monitor and record vital signs, weight, and fluid intake/output accurately.
• Report any changes in residents’ physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.
• Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.
