Qualifications

• Must have a valid Driver’s License issued by the Illinois Secretary of State.
• Proof of Auto Insurance is required based on individual circumstances.
• LPN or RN license must be obtained from Illinois Department of Financial and Professional Regulation.

Responsibilities

• Conduct assessments to determine the need for services for potential clients.
• Document all client activities accurately and in a timely manner.
• Make appropriate referrals to other agencies based on client needs.
• Communicate effectively with clients and professionals across various organizations.
