Qualifications
Must have a valid Driver’s License issued by the Illinois Secretary of State.
Proof of Auto Insurance is required based on individual circumstances.
LPN or RN license must be obtained from Illinois Department of Financial and Professional Regulation.
Responsibilities
Conduct assessments to determine the need for services for potential clients.
Document all client activities accurately and in a timely manner.
Make appropriate referrals to other agencies based on client needs.
Communicate effectively with clients and professionals across various organizations.
