Job summary

Mercyhealth Wisconsin and Illinois is seeking an APS Case Worker to assess needs, develop a plan of care, and link clients to services for adults 60+ or 18-59 with a disability in domestic or facility settings.

You will investigate abuse/neglect, document findings, coordinate with IDoA and waiver services, and participate in court testimony when required. Bachelor’s/Graduate degree or approved alternatives with three years of relevant experience are preferred.

Qualifications

• Bachelor’s or Graduate Degree in health or human services, social work, health care administration, gerontology, criminal justice, or nursing.
• An associate degree from an accredited program in health, nursing, social services, social work, health care administration, gerontology, disability studies, criminal justice or public administration and four years' experience in health or human services.
• An active licensed practical nurse license, with one year experience in health or human services
• Four years' experience with law enforcement or criminal investigation
• An approved waiver of educational requirements for intake workers and case aides through the IDoA
• Minimum three years of experience with assessment of needs for older adults (60+years) or those 18-59 who have a disability, preferred.
• The ability to negotiate difficult family situations using a variety of interview/assessment skills preferred.

Responsibilities

• Investigates abuse, neglect, abandonment, financial exploitation and/or self-neglect within IDoA standards.
• Develops case plans and links clients to resources; monitors outcomes.
• Provides monthly follow-up coordinating care/services and assessing risks.
• Completes IDoA/ Mercyhealth paperwork accurately and timely.
• Negotiates difficult family situations and involves supervisors when needed.
• Utilizes court system for Orders of Protection, Evictions, Guardianships; prepares and testifies in court if required.
• Coordinates with Waiver services per IDoA policies.
• Participates in M-Team meetings and records minutes for APS supervisor.
