Job summary

Catholic Charities, Diocese of Joliet is seeking a caseworker to provide direct support to abused and vulnerable adults and to document reports and follow-up actions. The role emphasizes dignity, respect, and timely responses within APS guidelines.

Responsibilities include coordinating care plans with the APS Supervisor, conducting follow-up contacts, and making referrals to community services. Strong communication and organizational skills are essential, with adherence to ethical standards.

Qualifications

  • Master’s degree or equivalent in health, social services, social work, health care administration, gerontology, disability studies, criminal justice or public administration.
  • Bachelor’s degree with 1 year relevant experience or associate degree with 4 years’ experience in health or human services.
  • Experience in health or human services and ability to relate well to persons with disabilities and older individuals.

  • Provide casework and follow-up services to substantiated victims, vulnerable adults, and verified abusers to prevent further abuse, neglect, or exploitation.
  • Receive and document reports of abuse, neglect, self-neglect or exploitation of older adults and persons with disabilities.
  • Conduct case consultations with the Adult Protective Services Supervisor for care plan development and case closure.
  • Provide follow-up services with face-to-face and phone contacts every three months for up to twelve months, longer when permitted
  • Document all activities on required forms, adhering to APS program policies and time frames.
  • Make referrals for appropriate follow-up services such as case management or community-based services.
  • Prioritize and respond to client needs within specified time frames.
  • Work effectively with co-workers and other community agencies.
  • Attend staff meetings, in-services, conferences, and workshops as recommended.
  • Be an active member of the APS Multi-disciplinary Team.

Requirements
  1. Master’s degree from an accredited university or college in health, social services, social work, health care administration, gerontology, disability studies, criminal justice or public administration; a bachelor of science or bachelor of arts from an accredited university or college in health, social sciences, social work, health care administration, gerontology, disability studies, criminal justice, or public administration and one year experience in health or human services; an associate degree from an accredited program in health, nursing social services, social work, health care administration, gerontology, disability studies, criminal justice or public administration and four years’ experience in health or human services; or four year experience with law enforcement or criminal investigations.
  2. Ability to relate well to persons with disabilities and older individuals, respecting their privacy and dignity.
  3. Ability to work effectively in a dynamic, fast-paced, and diverse environment.
  4. Ability to prioritize and respond to service requests in a timely manner.
  5. Proficiency in Microsoft Office applications (Outlook, Word, Teams).
  6. Strong verbal and written communication skills.
  7. Passage of complete background clearance and other required testing.
  8. Valid driver's license, reliable transportation, and proof of liability insurance.
  9. Adhere to the Agency’s Code of Ethics and conduct all responsibilities in accordance with the standards outlined in the Employee Handbook.
