Job summary

Griffin Health Brand is seeking a Nurse Practitioner with an RN background to lead wound care programs in Colorado. The role involves wound debridement, topical wound management, and coordinating related services at the point of care.

The ideal candidate has at least 1 year of NP experience and 5 years RN experience, with a WOCN certification preferred to enhance patient outcomes.

Qualifications

• RN license and NP license required.
• Minimum 1 year experience as an NP and 5 years as an RN.
• WOCN certification from the Wound Ostomy and Continence Nursing Certification Board preferred.

Responsibilities

• Debridement of wounds using surgical, sharp, enzymatic, and autolytic methods at point of care.
• Apply and manage topical wound care and related products.
• Coordinate referrals and ancillary services to improve wound healing outcomes.
• Interpret diagnostic tests and recommend follow-up actions.
• Prescribe necessary medications or DME within scope of practice per collaborating physician.
• Perform diabetic foot ulcer risk assessments per ADA guidelines.
• Educate patients on wound management and treatment options.
• Educate PCPs and referring physicians on current wound care practices.
• Provide updates to PCPs/referring physicians on patient status and systemic conditions.

Job description

Job Description/Specific Duties:

• Debridement of wounds by sharp, enzymatic, autolytic or surgical methods able to be safely completed in the place of patient visit and all associated procedures to complete the debridement.
• Application and management of topical wound care and associated wound management products.
• Direction and recommendation of associated services to improve wound healing outcomes. This includes but is not limited to labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
• Interpretation of diagnostic tests and recommendation of appropriate follow up.
• Recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
• Diabetic Foot Ulcer risk assessment for Members with Diabetes, as per the ADA guidelines
• Education of patients as to the topical and systemic management of their wounds.
• Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
• Provision of regular updates to the PCP, CM, or referring Physician on the status of their patient’s systemic conditions associated with wound care.
• Acts as a resource person for decisions, planning, and policies affecting the health of residents.
• Collaborates with staff to identify customer needs and to help develop plans to meet these needs.
• Other duties as assigned

Requirements

Registered nurse (RN) preparation and Nurse Practitioner (NP) license required.

Minimum of 1-year experience as an NP and 5 years’ experience as an RN.

Certification from the Wound Ostomy and Continence Nursing Certification Board preferred.
